Several alternative forms of ketamine administration—such as oral lozenges, nasal sprays, and intramuscular injections—have been introduced by newer providers. While these options may seem convenient, none of them have the extensive clinical validation that intravenous (IV) ketamine has accumulated over the past 30 years.
Hundreds of studies from the National Institute of Mental Health, Yale, Mt. Sinai, and other leading institutions confirm IV ketamine’s safety and efficacy in treating mood disorders such as treatment-resistant depression, PTSD, and major anxiety. By contrast, the safety and effectiveness of oral, nasal, and intramuscular ketamine remain far less understood and are not supported by the same level of rigorous data.
IM ketamine leads to a rapid spike in blood levels that can’t be easily controlled. This sudden uptake increases the risk of dissociative experiences that may be intense or uncomfortable. Without an IV line, there’s no way to administer a rescue medication—leaving patients to wait for the effects to subside on their own.
Nasal ketamine absorption varies widely and follows a different pathway than IV delivery. Because it’s routed through arteries that reach opiate-rich areas of the prefrontal cortex, the experience is distinctly different—and may increase the risk of repeated use or dependency, particularly in unsupervised settings.
This is why FDA-approved nasal ketamine (Spravato®) is only permitted for in-clinic use under direct supervision. While it may offer benefits for certain patients, it is not interchangeable with IV ketamine in terms of effectiveness or safety profile.
